Larssen proposed the name Lili, and by the 1920s, Elbe was frequently seen as a woman going places under that identity, attending events, and hosting visitors in her home. Gottlieb rose to fame as a result of her paintings of elegantly attired, stunning women with mesmerising almond-shaped eyes. Elbe served as the inspiration for these representations of diminutive femmes fatales. Following her shift, Elbe gave up painting.

Surgeries and dissolution of marriage

Elbe travelled to Germany in 1930 to have very experimental sex reassignment surgery. Over the course of two years, a series of four surgeries were performed. Erwin Gohrbandt removed the testicles in the first operation, which was carried out in Berlin under the direction of sexologist Magnus Hirschfeld.

Kurt Warnekros, a physician at the Dresden Municipal Women's Clinic, performed the remainder of her operations. In the second procedure, an ovary was implanted into her abdominal muscles, and in the third, the penis and scrotum were removed.

Her situation had already made headlines in German and Danish newspapers at this point.

After the couple's marriage was declared null and void by a Danish court in October 1930, Elbe was able to officially change her name and sex as well as get a passport under the name Lili Ilse Elvenes. Danish writer Louise "Loulou" Lassen of Copenhagen initially adopted the pseudonym "Lili Elbe" in an article for Politiken in February 1931. Elbe went back to Dresden and started seeing Claude Lejeune, a French art dealer, with whom she hoped to get married and have children.

She underwent her fourth surgery to implant a uterus and create a vaginal canal in 1931. A few weeks after Dora Richter underwent the experimental treatment, she became one of the first transgender women to have a vaginoplasty surgery.
